Apply for Funds
Applying for funding from the Club of Hearts is an easy four-step process:
Step 1- Read the guidelines (PDF 2 MB).
- Complete the agency application form (PDF 2 MB) and click the SUBMIT button at the bottom of the form.
Note: You can complete the file online and submit it online. However, you cannot save the data typed into the form. Please print your completed form if you would like a copy for your records.
Step 3- Click here to attach and send your IRS tax-exempt letter.
- Click here to attach and send your IRS Form 990.
IMPORTANT: When you send the required documents in Steps 2, 3 and 4, you will NOT receive a confirmation statement. Please check the SENT box in your email to ensure the documents were submitted.
