For more than 50 years, the Club of Hearts has contributed millions of dollars to health and human services in the Atlanta area. If you are a 501(c)(3) organization wishing to receive funds from the Club of Hearts, follow the application process outlined below.
Applications are accepted only in the month of March. The application deadline is March 31, 2013. Applicants will be notified of the outcome of their application within four weeks of submission of the complete application package.

Complete the agency application form (PDF) and click the SUBMIT button at the bottom of the form to submit your form via email.
Note: You can complete the form online and submit it online. However, you cannot save the data typed into the form. Please print your completed form if you would like a copy for your records.
IMPORTANT: When you send the required documents in Steps 2, 3 and 4, you will NOT receive a confirmation statement. Please check the SENT box in your email to ensure the documents were submitted.
